I-693 PAPERWORK

Our physician, Dr. Hong Xiao, is certified to provide USCIS immigration physicals. We are currently accepting new patients for the purpose of completing the paperwork necessary for immigrations, green cards, and etc. All physicals are on an appointment basis and must be scheduled before the patient arrives. This can be done by calling our office.

PROCESSING TIME

The USCIS packet can be completed in two visits, with 1 week in between.

First visit:

This visit includes a meeting with the doctor, bloodwork to determine antibody levels for vaccines, a TB test, and a urine test.

What Should I Bring To The Exam?

Having all of your documents ready before your appointment can help the medical exam go smoothly. Here’s what you’ll need to bring with you:

Your immunization or vaccination records
A copy of your medical history
Copies of any previous chest X-rays, if any
A letter from your regular doctor outlining the treatment plan for any health problems you have
A government-issued photo ID, such as your passport, state ID, driver’s license, or work permit.
Payment for the medical exam fee
Your health insurance card, if any (check with our office before your appointment if we accept your insurance)

The following vaccinations are required for immigration purposes:

COVID-19 (Required 2024 booster)
Hepatitis A
Hepatitis B
Influenza (only during the Flu season)
Measles
Meningococcal
Mumps
Pneumococcal
Pertussis
Polio
Rotavirus
Rubella
Tetanus and diphtheria toxoids
Varicella

(Your bloodwork will test for antibody levels of these vaccines, except COVID, Tdap and Flu.)

Second visit:

Once lab results have been received, patients can come in for the follow up visit. This visit includes vaccinations and an x-ray if necessary (for an additional fee). Upon completion of this visit, the patient will receive a signed and sealed packet, ready for submission to USCIS, as well as a copy of their paperwork.
